When I enter a substitute pitcher in the middle of an inning (i.e. pitcher B subs Pitcher A in second inning on Batter #4) the software is indicating that I have two pitchers playing at the same time. What is the correct way to enter the sub pitcher?

You can specify exactly when the substitution was made by right-clicking on the pitcher and going to 'substitution details'.
